Ecuador Mission Trip - July 16-25, 2015
Thank you for your interest in the Ecuador Mission Trip (EMT)! This trip is for students in grades 9-12 during the 2014-15 school year. Please follow the steps below in completing the application process. Submissions due by December 21, 2014. Thank you! (Note: Each team member is required to have a passport. See link to right.)
Step 1: Required - Read the informational letter from Pastor John. Click here to access the letter.
Step. 2: Complete the Application and submit it online or hard copy. See "Complete Application" below.
Step 3: Print out, read, complete and turn in the Medical Consent form. Click here to download and print the form.
Step 4: (Only for those who have not been on a mission trip with Pastor John) - Click here to access the online reference form or click here to download and print a copy. There are THREE references required: One from a parent, one completed by you, and one from a non-family adult who knows you well.
Step 5: Turn in your deposit check of $200 to Pastor John.
Read the letter, complete & submit the application, turn in your medical consent form, turn in references (if required), and turn in deposit check to Pastor John no later than December 21, 2014.
